Care Collaborative ACO

The Care Collaborative is an accountable care organization (ACO). The ACO consists of doctors, hospitals and other healthcare providers who choose to come together to provide coordinated, high-quality care to Medicare patients, and share in the savings generated by improving their outcomes.

The goal of coordinated care is to ensure that patients get the right care at the right time, avoiding unnecessary duplication of services and preventing medical errors.

When an ACO succeeds in delivering high-quality care and spending healthcare dollars more wisely, Medicare shares the resulting savings with the ACOs.
