Care Collaborative
The University of Kansas Health System Care Collaborative is a network of healthcare providers and care teams dedicated to ensuring rural patients in Kansas have access to the same high-quality care as patients in urban areas. Together, collaborative members work to improve the health of people living in rural and urban communities, using evidence-based models of care to improve outcomes and decrease costs.
Health system physicians and other clinicians collaborate with care providers across the state through a network of hospitals, clinics and EMS teams in 81 of Kansas’ 105 counties. The Care Collaborative is comprised of 99 provider organizations, including hospitals in 72 of the 75 rural Kansas counties with hospitals, along with their affiliated clinics.

The Care Collaborative was founded in 2014 with a CMS Innovation Award to build a rural clinically integrated network. Originally known as the Kansas Heart and Stroke Collaborative, it started with 13 rural hospitals implementing evidence-based practices for time-critical diagnoses and delivering chronic care management services. Since then, the Care Collaborative has grown both in the number of participating providers and the scope of its work.
