Overview of Your Executive Health Appointment
After your appointment is scheduled
We will provide an individualized health risk assessment questionnaire to you for completion to gather information regarding your medical history. Once complete, a member of our team will contact you to review your medical history and answer any questions you may have. Based on this feedback our dedicated nurse navigator will customize an executive physical visit that is tailored to your individual needs and health risks.
The day of your physical
You will meet with our board-certified internal medicine physician who will provide you the details surrounding your appointment and answer any questions you may have. During the remainder of your physical evaluation, our team will collect the necessary lab work and conduct preventive screenings to assess your overall health and detect conditions including cancer or heart disease.

Prepare for your appointment
To prepare for your visit to The University of Kansas Health System executive health program, we recommend:
- Schedule your executive physical by calling 913-574-2880.
- Upon scheduling your physical, an individualized health risk assessment questionnaire will be provided to you for completion.
- Complete phone consultation with our dedicated nurse navigator to design a customized physical based on the review of your medical history and any associated health risks.
- Receive your customized itinerary and care instructions for the night before and day of your physical.
- Bring to your appointment:

- List of questions

After your physical examination
You will meet again with the physician to review your clinical results and discuss any additional screenings and lifestyle recommendations. You will be able to access your medical information online in MyChart, our online health record portal.
