Clinic Services Authorization

Employee clinic service authorizations can be emailed directly to the clinics at OCC@KUMC.edu. Please specify if you would like to schedule an appointment or if the employee will be walking in for services. Include the company name, employee name and authorized services in the email. The employee must bring a form of identification.
