We offer a range of family medicine services for young and old alike as well as physical therapy rehabilitation and laboratory services. Our family medicine providers help with the prevention, management and treatment of common and complex medical conditions for people of any gender and age, from infants to older adults. We offer same-day appointments. Our physical therapists work with you to achieve long-term health benefits. We work with inpatients and outpatients at convenient locations throughout the area to provide evaluation and rehabilitation services for optimal healing and improved quality of life. From I-435 W., take Exit 81 to merge onto US-69 S.
Take the KS-68 exit toward Louisburg/Ottawa.
Turn left onto KS-68 E./K-68 Hwy. E.
Turn left onto E. Crestview Drive.
Turn left, then turn right again and your destination will be on the left.
Free parking is located directly in front of the building. Enter through the double glass doors on the east side of the building and check in at the registration desk.
