What to expect on your first visit

Patients come to the Alzheimer’s and Memory Clinic from a five-state region for the expertise and counsel of our care team. On your first visit to the clinic, you can expect a complete review of your medical history, memory testing and a neurological examination.

How to prepare

After making your appointment, you will receive in the mail a health history form and insurance information to fill out and bring to your first visit. If you have had lab work or images taken at another doctor’s office, remember to bring that information or have your doctor send it directly to the health system. Your first visit will last approximately 90 minutes to two hours. It is important that you are accompanied by a person who knows you well – someone who knows your daily routine and can answer questions about it.


  • Physician referrals

  • Patient self-referrals
    We also encourage prospective patients to contact us directly, removing the barriers to your receiving a diagnosis and treatment. To request an appointment, call 913-588-1227.

New patient packet

New patients receive literature that explains the disease and gives information to patients and families about living with Alzheimer’s disease and increasing the quality of life.
